Forms are a simple and effective way to collect and share important information using your walt radio and mobile app. Whether you're reporting an issue, answering a survey, or logging data, this guide will walk you through how to find and complete a form quickly and accurately.
How to Find a Form?
There are a couple of ways a user can access a form.
From the walt Menu

Tap the three-bar stack icon located in the bottom left-hand corner of your screen or click the back button to get to your Walt menu.

From the menu, select Forms.
If you're on your mobile app, navigate to your menu in the bottom right hand corner.
Pro Tip! Don't see the forms option? Contact your supervisor.
Once you click into forms, you will see all of the forms you have access to.

From a Form Link Shared in a Channel

Sometimes your admin may send a form directly to a channel you’re in.

Just click on the link provided in the message. You can also use the buttons to navigate up into the messages and click "O" to select.

The form will open immediately, and you can begin answering the questions.
By scanning a QR code
there's an option at the top of the forms list to "Scan QR code"
Answering Form Questions
Once you select the form you would like to complete, complete the questions that are presented to you. Below are some examples of question types you might see.
- Multiple Choice: Select your answer from a list of options.
- Push to Talk (PTT): Hold down your blue PTT button to record your voice response. Might call out here that there is a keyboard on the lower-left, where they can type or correct the PTT input.
- Number: Use the keypad to type in a numerical answer.
After you've answered the last question:

You’ll be asked if you’re ready to submit.

Tap the green checkmark to submit your responses.

Tap the red X to go back and edit any answers before submitting.
