Forms used to be boring, right? Endless pages of paperwork, crumpled edges, and endless fields to fill in by hand. But not anymore! Enter Digital Forms—a game-changer for anyone who craves simplicity, customization, and, let’s be real, way less paperwork. Whether you’re gathering feedback, collecting data, or onboarding new team members, these digital forms are as flexible as they are fun to create.
What’s even better? You can make them unlimited! Yup, there’s no cap on the number of forms you can build and customize to fit your exact needs and distribute them to just the right group of workers. Want a maintenance ticket complete form that is just used by Maintenance? Easy!

Important things to note before we start:
- To receive the results from your Form, you can access those directly in your console. You also have the ability to export the answers!
Getting Started
The first thing you are going to want to do is navigate to your weavix Console. Once in the Console, you are going to want to select "Forms" under your Configuration tree. Now that we are in the Forms section, please follow the steps below to build out your customized Form.
Creating a New Form
You will want to select the big blue "Add Form" button in the top left-hand corner of your screen.
Once selected, you will then be prompted to name your Form, and also assign which site your Form can be used at. Users that have Permissions at the assigned site, will be able to access the Form.
Pro Tip! Please assign your Form a Site even if you only have one Site on your account. This will ensure that as your account grows, your Form will stay with your specific Site. This will also only allow users who have permissions on your site to have access to the form.
The next box you will want to make sure is highlighted is the "Accepting Responses" checkbox. This will allow users to submit answers to the form. When a Form is disabled, you will still have access to it in the Console, but your users will not see it on their walt Smart Radio.
Pro Tip! When you are done collecting answers from the form, simply uncheck the "Accepting Responses" checkbox. This will remove the form from users' devices until you are ready to use it again.
Form Access
You can manage who has access to each Form. To limit a Form to a specific group of users, you will need to select the blue plus sign on the right-hand side of your screen. You will then need to work through the drop-downs, customizing who you would like to include or exclude from the Form.
In the example below, you can see that only people with the safety craft has access to submit answers to this Form. To ensure the correct users have access to the form, click the View Selected Users button on the right.
Pro Tip! If a user is not allowed to submit a form response, they will not see the Form as available in their list.
Pro Tip! You also want to make sure the users permission group is set up to allow them to submit forms. You can reference this article to review the settings of your permission groups.
Creating Questions
Once you have your Form named, the Site assigned, your "Accepting Responses" checkbox marked, and who you want to have access to the form determined on the General Page; you will then select Questions on the top of your page.
To add a question, you will need to click into the "Add Question" text box. You will then be pulled into a screen that prompts you to fill out the details of your question.
On this screen, you will need to type your question and select how you want the user to answer the specific question with the "Type" drop-down.
- Company Picker: Allows you to select a specific Company
- Date: Allows you to select a specific Date
- Dropdown: Allows you to pre-fill answers and allows the User to select from said answers
- Item Picker: Allows you to pick from existing Items within a specific Site
- Number: Allows the User to input a Numeric field
- Person Picker: Allows you to select a specific Person on a Site
- Text: Allows a User a free-form Text field to input their own response
Pro Tip! Please see our example below with a "Dropdown" question with multiple values.
To add additional questions, you will need to select the plus sign on the far right-hand side of your screen and repeat the same steps.
Once you are done adding all of your questions, please select "Save" in the bottom right-hand corner.
Pro Tip! Your Forms can then be accessed on the walt Smart Radio or mobile app to be filled out.
Sending Form Link
Need a quick way to distribute a form to multiple users? Once your Form is created, you have the option to send a link to the Form directly in a channel for your users to fill out. You have the ability to copy the link from console or the mobile app!
Console
Navigate to the Forms tab in console.
Click the checkbox by the form you would like to share.
In the top right corner, click the Download Form Results button.
You will then see a Copy Link option.
You can now paste the link into any channel using the weavix web radio.
Mobile App
Open the weavix app and tap the menu icon in the bottom right corner.
Select Forms from the menu options.
Find the form you want to share and press and hold it.
Tap the Copy Link option when it appears.
Navigate to your desired channel and paste the link to share it with your team.
Once the Form is sent to a channel, users can complete the form from your walt radio or mobile device.

Pro Tip! You can send a Form link from the web radio, but users are not able to fill out the Form from their web radio. They must log into their mobile app or Walt Radio to complete a Form.

Download Form QR Code
You can easily access and share forms using a QR code. Each form has an option to download a QR code that can be scanned with your mobile device or walt smart radio. Scanning the QR code will take you directly to the form, making it easy to view and fill out.
Click into the form you want to download the QR code for.
Then click on the blue Download Form Results in the top right-hand corner. You will then see a Download QR code option. A QR code will be downloaded to your computer, which you can easily share with users or print and display around your facility.
Pro Tip! Place a QR code directly on machine or equipment to give users instant access to a maintenance request form. It's a quick and efficient way to report issues right on the spot!
Download Form Results
Need to view the current results of a form? You can easily download those from your console!
Click into the form you are wanting to download the results for under the Forms tab.
Then click on the blue Download Form Results in the top right hand corner. You will then see the option to Download Results. This will download all of your form results into a .csv excel file.
This export will show the date and time the form was completed, the name of the person who submitted it, and their responses to all form questions.
Pro Tip! If users are on their walt radios, they can respond to open-ended questions using their push-to- talk button. Their voice responses will be automatically transcribed and included in the export. (See example in Column E.)
